Patient Coordinator Remote Monitoring (Wearable Devices)

Location: Remote / Hybrid in metro cities

Role Overview

As a Patient Coordinator, you will be the primary point of contact for patients enrolled in our remote monitoring programs. You will guide patients in using wearable devices, monitor incoming health data, provide proactive outreach, and escalate clinical concerns when needed.

This is a patient-facing, tech-enabled clinical coordination role that blends compassion with data-driven care.

What Youll Do

  • Onboard patients into remote monitoring programs.
  • Educate patients on wearable device setup, usage, and troubleshooting.
  • Monitor daily incoming health data (e.g., vitals, alerts).
  • Identify abnormal readings and escalate per clinical protocol.
  • Conduct proactive follow-up calls to improve adherence and engagement.
  • Document all patient interactions in the EHR system.
  • Collaborate with physicians, care managers, and technical teams.
  • Contribute feedback to improve workflows and patient experience.

Who You Are

  • Registered Nurse (RN) / BSc Nursing / GNM with valid license.
  • Strong clinical assessment skills.
  • Excellent verbal and written communication.
  • Highly empathetic and patient-centered.
  • Comfortable using digital tools, dashboards, and wearable technologies.
  • Self-driven and able to work independently in a fast-paced startup environment.

  • 85% patient device adherence rate
  • Average patient response time 24 hours

2 Clinical Monitoring & Escalation

  • 100% adherence to escalation protocols
  • Alert review within defined SLA (e.g., within 2 hours for high-risk alerts)
  • < 2% missed or unreviewed alerts

3 Documentation & Compliance

  • 100% accurate and timely documentation
  • Zero major compliance violations
  • Audit score 95%

4 Patient Experience

  • Patient satisfaction score 4.5/5
  • Low patient drop-off rate (< 10%)

5 Operational Efficiency

  • Ability to simultaneouslymanage multiple locationpatients
